This form is for one child at a time. If you have more than one child who needs shoes, submit this form again for each child after this one — it only takes a minute, and it's the only way to make sure each child's request is entered correctly.
A few things to know before you submit:
  • Every request is reviewed before it appears publicly.
  • The response has been bigger than we imagined — requests are reviewed and posted in stages, and submitting doesn't guarantee assistance.
  • Requests may be prioritized by timing and available resources.
  • Your private information is never shown publicly. The board only ever shows a size, a category, and a request number.
  • Leslie's Laces provides shoes only — not rent, utilities, food, or other assistance.
