This form is for one child at a time. If you have more than one child who needs shoes, submit this form again for each child after this one — it only takes a minute, and it's the only way to make sure each child's request is entered correctly.

Every request is reviewed before it appears publicly.
The response has been bigger than we imagined — requests are reviewed and posted in stages, and submitting doesn't guarantee assistance.
Requests may be prioritized by timing and available resources.
Your private information is never shown publicly. The board only ever shows a size, a category, and a request number.
Leslie's Laces provides shoes only — not rent, utilities, food, or other assistance.
